    The switch travel, from momentary to click-on, sometimes breaks contact... resulting in unintended mode changes. It was worse brand new out of the clamshell package then it is today. I suspect with more use and switch cycles it may cure itself. I purchased the light for it's high mode anyway...

    Your shape description reminds me of Osram Sylvania Capsylite, though I don't recall ever purchasing a rough service version, and I haven't seen any in stores for years. The popular search engine brought up a part Sylvania 42MB/CAP 130V, but I am not familiar with the seller.

    I'm not an electrician. Perhaps you have a "rapid-start" ballast instead of an "instant-start" ballast. I've seen some with as few as 3 output wires, using shunted tombstones, and maybe that is what your LED T8 tube is expecting.

    The 2 cr123 primaries you are using are the original ones that came with your Surefire, correct? Some people find their lights only whine on partially depleted cells, or certain modes, etc. If you had other brand new cr123 primaries to try your drop-in may (or may not) be silent. This ^^
