I just stumbled upon the following report of an unfortunate San Diego RC hobby shop that burned down after the owner (Jim Bonnardel) stepped out for 10 minutes while charging a LiPo on Jan 16, 2015. Alas, even folks with decades of experience can make mistakes. The first link has a photo and video, and a brief description of the circumstances.
