I think I know what the answer is. I just want to hear some ideas while I wait for the replacement.
I bought a couple of glass lenses from Flashlightlens.com, One for a Novatac 120E and one for a Nuwaii Q3. Installed both. Both lights are subject to the same pocket carry as many other glass lenses are. The Q3's lens has a crack. From the edge to the center.
I think, I either tightened it down too tight when I put the reflector back in or the o-ring was toast and allowed me to pinch it or bind it on assembly. Neither have been dropped since the lenses were installed.
Or it could be that rare bad lens.
