Sorry if I caused confusion- the Olight S1R Baton takes the positive towards the tailcap, so I inadvertently inserted my 18650 into the D4VN with + terminal at the tailcap, causing a short circuit that almost led to a catastrophic event. I was sharing this story, as I usually am smarter than the equipment I work with....illustrating that it is always wise to never let fatigue, complacency or both cause an error that could turn an awesome flashlight into a not so cool pipe bomb.....

Not to say that it wont or could not happen but I think you are been over dramatic... IE: https://www.youtube.com/watch?v=M-iQiqlQBtE
So saying "pipe bomb" goes far out... Again not saying that it could not happen but you could use better description or analogy's...
Oh and there is more discussions of this all over the internet, none which I found to explode...

There have been a few documented cases, including at least one with video here on CPF, of a venting with fragmentation. Of the flashlight; however I agree that my description was more dramatic than I realized or intended. The feelings immediately after the incident remind me of the saying that “when your up to your eyeballs in alligators, it is hard to remember that the objective was to drain the swamp. In all seriousness, this was intended as a reminder to stay on task and use caution...I would hate to discourage anyone from the use of Lithium-ion batteries because of my mistake, it was merely a reminder to use caution...
