How does a Lithium AA battery work?

What's the deal? Is there a resistor inside to lower the output voltage?

Is there a mini-transformer to lower the voltage?

What's the deal? If it's Lithium, there's a 3+ voltage in there somewhere. When it gets trimmed to 1.6 or so... How?

No, it use a different formula then those 3V lithium.

AA L91 is Lithium Iron Disulfide (Li/FeS2)
123 is Lithium Manganese Dioxide (Li/MnO2)
