Have an MDC 3.4-6V head. It runs well on a 18350 of course. Don't have a 2x123 body but wonder how well it will run on a single 123 primary.
I was able to get it to fire on one but wonder at what voltage it cuts off.

I also have the MDC 2.3-3.2V head and that should run better than the above on 1x123 but just wondering. Because I am not sure I will always run 18350, at times it's nice to run 123s.

Also, the MDC 3.4-6V head seems to run OK on Energizer L91 Lithiums and I just tried alkalines, they work. Question is, for how long? Eneloops don't fire up this module at all.

What 18350 body do you use with the Malkoffs?

The vf (forward voltage) of the LED will determine where it cuts off.
Some LEDs will need 1.5v, some 1.8v, and others 1.2v...just all depends on the LED but generally 1.5v is it.

A 1.5 alkaline working but a 1.2 Eneloop not working tells us around 1.5v is the LED cut off on the MDC 3.4-6v.
I can't imagine a 1.5 alkaline lasting too long before it drops around 1.2v, especially under a load.
The higher the current the more voltage gets drained.
That's why the beloved LLLL lasts forever.

A custom made 18350 body that I want to run either full power 3.7V 18350 and sometimes 123.

with the MDC 3.4-6V head, of course I meant the 2xAA body with two L91s in it. They does power it on. So do Alkalines. It's obviously under-driven.

I was aware of the thread, have it saved. I missed the fact he did the MDC 6V head. Nice. This is not exactly what I wanted to know. It's 6V head on high. What about low? would like to see the 3.4V -6V MDC head on 123 on low.
