Are burnt out LEDs common?

I've seen a bunch of threads recently where people are reporting that their LEDs are now showing burnt out brown spots in the middle after a few years of use which can't be anywhere close to 50,000, 10,000, or even 1,000 hours.



I've heard that typically overdriving an LED can cause burnout. Are LEDs commonly being overdriven in most flashlights on max mode? I've noticed that Fenix no longer seems to have the warning on having turbo on for more than 10 minutes for its LD10/20 and others like Inova, 4sevens, etc. don't seem to have a warning for max at all.



Does running a flashlight on max rapidly burn out an LED? If so, which models and makers does this affect?
