Need a tight spot in a body like Fenix MC11

I am an ENT doctor and have cobbled together my own LED headlights for the past 10 years and technology has finally progressed to the point where the MC11 gets pretty close to what I need but not quite.
Before this I was using a Pelican 2250 Versabrite and chopping the head off and then wiring in a AA MiniMaglite LED head. The Versabrite body fails after about 3 months of use.

I need something with an extremely focused beam. Ideally with all the light focused into a 2-3" sized area at 18 inches from the light.
Is there anything out there that does this or is there a place where I can get the parts to put one together?

I can buy a "medical" headlight for $300 but it has an external battery pack which is just crazy with today's technology.
