Not sure if this is helpful but it has all the info that I look at!
I have it all on one sheet and keep a copy at home, work and on my phone.
It usually prints out straighter than below :) I had to put the underscores in as the tabs kept getting removed.

just out of curiosity, are you sure 18650 can be charged on 4.5A? I've read somewhere that only 3400MaH 18650s can be charged on 2A and the rest at 1.5 or less. I might be wrong though
