Runs on: 2 x CR123A Cells or 1 x 18650 Battery ...

Lots of lights have this spec, implying they can handle a voltage range from 3.6V to 6V.

If 2 x CR123 works, why not 2 x RCR123 (16340)? Does the voltage of about 6.4V with two RCR123s become too high?

And how much brightness or efficiency does one lose by using an 18650 vs 2xCR123? Does it mainly depend on the circuitry of the light?

A 16340 off the charger has about 4.2 volts which is 8.4 volts total which is too much for some drivers. Other lights/drivers are listed up to 9 volts. If I remember right, I had a Jetbeam Jet III-M that could handle up to 18 volts.

Using a quality 18650 will increase runtime and be cheaper in the long run compared to CR123's.

18650 are a bit of pain to start, the batteries, the charger, (always be safe), but its really worth it. Saves money in the long run, and much longer run times and most lights do better with the 18650 compared to the other battery configs in my experience.
