3-up MPCB identification

Can anyone identify this MPCB and tell me where I can get it without LEDs mounted?

Perhaps send them an e-mail and ask them if they carry unpopulated versions of that. I kind of wonder if maybe they had that made for them, i.e. if LT means LED-Tech

They don't offer them as one-offs but offered to solder in whatever LEDs I want
