We just shorted out a custom made 3 pack of AAA Eneloops at work:

No-one was hurt and the cells fizzed and popped for a while but didn't explode or anything like that. Nasty smell, as expected. At least we know they are safe now!
 
Not much to say really. Prototyping some hardware, guy was assembling a pack by taping the cells together, soldering the tags and adding wires and a thermistor. While the connector was being assembled the ends shorted together and the wires rapidly melted, causing a further short.
