I found this nifty lecture titled
Lithium-Ion Battery Safety Study Using Multi-Physics Internals Short-Circuit Model
at this link (opens PDF file):
http://www.nrel.gov/vehiclesandfuels/energystorage/pdfs/45856.pdf

Sadly, these are only the slides. It takes some patient study to understand them. Even then, a couple of the slides are still rather obscure to me.
I found the following pages to be particularly interesting:
page 3 = lecture thesis
page 21 = separators
pages 22-24 = mechanisms for electrode-to-electrode shorts
pages 28-29 = summary
And, yes, it took some staring at the 3D thermal graphs to understand them.

On a similar note, I found
A General Discussion of Li Ion Battery Safety
at this link (opens PDF):
http://www.electrochem.org/dl/interface/sum/sum12/sum12_p037_044.pdf
The most interesting part (besides the cool pics) was
Cathodes in Lithium Ion Batteries, on page 5.

Some of the statements in these articles sound familiar, so these might already have been cited at CPF. However, I found them to be valuable enough to repeat. I found them to be excellent primers (well, OK, they are rather technical).
