Anyone ever make one of these other than the factory surefire ones?

I've got an old surefire e1b that I'm pretty fond of but it could use a more modern head, don't want to give up the
low/high modes though. I'm coming up empty google'ing around. I think one could use the Malkoff P60 adaptor
and a M361 but it would be out of regulation on a single cell which does not sound ideal.

As above, Lumens Factory is a good option. Also Malkoff MDC series has multiple modes. Tana Workshop also does custom heads and drop-ins with programmable modes.

The Oveready BOSS E-Series Scout head would work, if you can find one of these out-of-production gems. Otherwise, I agree to trying Lumens Factory. Look at their E-to-C head and their D26 dropin that is designed for 3 to 3.7 volt input.

Just as an update, Malkoff does not offer the 3 mode MDC light head seperately from his single cell complete light on the site but I emailed and asked nicely and they sold me one. Note they have two versions, one that starts on high and moves down and one that starts on low and moves up. Very happy with it on my little old E1B.
