Overdischarge, from what I read, starts metal crystals that grow slowly, and eventually get big enough to break the internal membrane -- then the chemical reaction heats up. That doesn't require a charged battery at all.
The only lithium-ion battery I've had fail would not take a charge at all -- an old 3.7v CR2 that had many cycles.
I set it aside, checking the voltage (zero) several times over several days.
Weeks later I looked found brown discoloration all around each of the little holes around the positive terminal;
it had eventually broken down internally and vented something.
