Rechargeable LED flashlights?

I've been googling around for the last day or so trying to find a rechargeable LED flashlight with the following characteristics:

LED
Must be rechargeable/self contained...Ie, I don't want to remove the batteries to recharge, it should plug in to the AC adapter or be in a cradle.
Must be able to leave battery on charge all the time, and not have to take it off charge when it's full.
Should be rechargeable either via AC or 12v vehicle systems
Should have tail cap switch w/momentary on/remote switch capable would be nice too.
Should have fairly high output, 60+ lumens
Should be able to work in fairly cold (ie 0 F) weather
Should be 2 or 3 AA in size (may consider a C size if nothing else matches)

Any thoughts?

I don't know that you can leave it plugged in and charging all the time but the ITP R01 is a decent light.
 
To me 40 degrees is extreme cold so can't help with that.But the Streamlight strion LED meats all your other needs and is a really nice light.It's the issue light for some major PD's around here.
