It seems like lens rattle is a somewhat common issue with those things, even when the Maglite's head is screwed all the way down. Apparently, TerraLUX had to start shipping thicker O-rings with their drop-ins.
But I notice http://www.flashlightlens.com/ sells 2.84mm thick lenses for use in D-cell Maglites without a reflector. Sounds like that would be a very quick, easy, cheap way of eliminating any possibility of a rattle (even use it with the thicker O-ring if necessary), while also helping keep the bulb in the socket, and increasing your light output by 5% or more.
Anyone wanna try it? I don't have one, and no plans to get one any time soon. It'd be kind of redundant with a Magcharger and TLE-300MR on the way.
